Usher Syndrome

Requirements for Inservice Points

Inservice points can only be awarded by a Florida school district/organization with an approved Florida Master In-service Plan (MIP). It is recommended that you contact your individual school district's professional development department BEFORE beginning a course, to verify the requirements you must fulfill to earn inservice credits towards renewal of a Florida educator's certificate.

To be eligible for possible district-awarded inservice credit for this course, you must successfully complete all of the following items:

  • Review of All Course Content
    Review all the course content, including additional resources, external links and videos. Pass the Check Your Knowledge quizzes at the end of each unit.
  • Final Assessment
    Pass the final assessment with at least 80% accuracy. The final assessment can ONLY be accessed AFTER you have reviewed all the course content and is located under Required Next Steps on the My Courses page.
  • Follow-up Activity
    Complete the follow-up activity by applying what you have learned in this course.
    • Select a student from your district and complete an Usher syndrome screening using the steps outlined in this course. Write up your results in a narrative format explaining why the student was considered low or high risk.

    Complete instructions can be accessed under Required Next Steps on the My Courses page. They are ONLY available after you have successfully passed the final assessment.

  • Follow-up Activity Verification Form

    Submit the Follow-up Activity and Verification Form to your district contact person to review and sign. If you do not have a person in your district that is familiar with the Usher syndrome screening process, email you follow-up activity and form to RMTC/DHH at info@rmtcdhh.org.

    Upload the signed verification form to the My Courses page under the Required Next Steps.

  • Satisfaction Survey
    Complete the Satisfaction Survey. It is only available after you have completed ALL the above requirements and can be accessed under Required Next Steps on the My Courses page.
  • Certificate of Completion
    Print and submit the Certificate of Completion to your professional development department. It is ONLY available after you have completed ALL the above requirements and is located under Required Next Steps on the My Courses page.
